We have a number of highly successful programmes to assist with the care of people who have chronic or long-term conditions like diabetes, asthma, COPD, heart problems, gout, depression etc.
How we can help
Care Plus
Care Plus is a full check-up and support service for enrolled patients with more than one chronic or long-term condition, acute medical or mental health needs or terminal illness.
Care Plus is provided through Counties Manukau District Health Board and offered by all Local Doctors and White Cross GP clinics.
If you are eligible for Care Plus you are entitled to cheaper clinic visits four times a year. Ask your doctor or nurse if you think you may be eligible. If you are not, other support and services may be available.
As a Care Plus patient you will receive:
- an initial, full assessment, exploring your health needs
- an individual care plan including realistic, achievable health and quality of life-related goals, with regular follow-ups
- effective management of chronic health conditions
- a better understanding of your condition
- support to make lifestyle changes.
Self management support
We offer programmes to help eligible patients manage chronic conditions or high-risk health problems, including:
- diabetes
- cardiovascular disease
- congestive heart failure
- chronic obstructive pulmonary disease
- gout
- asthma
- depression
- multiple chronic conditions.
Patients can also receive on-on-one health coaching. The programmes are free, or available at the subsidised rate, to eligible patients.
A nurse provides ongoing support, a care plan with personalised goals, resources and advice for navigating the healthcare system. Transport can also be arranged to get you to your clinic.
Self-management education
Self-management education is a free programme in your local community for you and your family. The programme improves wellbeing, especially for those who have long-term medical conditions.
The programme helps you:
- learn new skills to manage your health condition
- develop confidence in the way you manage your condition(s)
- meet others and share similar experience
- learn new tips for keeping as well as possible
- feel in control of your life
- have the opportunity to be trained as a community leader
- achieve goals and milestones
- turn obstacles into milestones.

How long will I have to wait to see a doctor?
Most people in this country have to wait to see their doctor! That’s because people have different health needs and how long it takes to get the right help can be very unpredictable. It’s our long-held philosophy that you should be able to see a doctor on the day you need to – and not have to wait for days.
The most seriously ill people are seen first. The really good news is that you can now see on the clinic website pages what the expected wait time is at that clinic! That means you can find another clinic near to you with a shorter wait time, if need be, or choose to come another day.

What is ManageMyHealth and how do I join?
ManageMyHealth is a secure website that can receive your health information from your doctor’s system. You can use it to store other health information, too, such as other treatments you have received or other medications that you may be taking. This means you can access your personal health information from any device at anytime, anywhere in the world. If your clinic takes appointment bookings you can do this through ManageMyHealth as well as order repeat prescriptions. Your online Personal Health Record can be made up of:
- Consultation notes
- Laboratory results
- Prescriptions
- Immunisations
- Allergies
- Diagnoses
- Recalls.
Just ask the receptionist at your local clinic to sign you up to ManageMyHealth.

Where do I get my test results from?
When you have a blood test, or any other kind of test, your results will be sent to your clinic. If the results are normal, you will not be notified. If they are not normal, the clinic will phone you and discuss next steps. If you have a ManageMyHealth patient portal, you can find your results in there, once your doctor has reviewed and filed them. Or, if after two or three days, you are concerned about your results, you can call the clinic to find out.

How do I order a repeat prescription?
If you have a ManageMyHealth patient portal, you can order a repeat prescription online, otherwise please visit your clinic’s page and phone them.
